Would be most effective if it could also remove the "recommendations" and "latest" ribbons from the home page, as well as being extended to actual YouTube video pages as well as on search, history, and watch later pages.

First, visit a YouTube video. Second, "Request Desktop Mode". Third, copy the url. Fourth, uncheck "Request Desktop Mode" then paste the url. This is situational and will only work on that one video.
Fifth, click onto another video the new layout should be restored. Sixth, now select Youtube's desktop mode, you can get to this by pressing the three dots near the video.

YouTube (I'm using the U.S. site) seems to have been changed earlier today so that having your youtube.com PREF cookie contain "f6=8" no longer reverts to the pre-Polymer UI. I tried the addons Old Youtube Layout, Restore Old Fast Theme of YouTube, Switch for YouTube, YouTube Classic (the only one with a recent update), and this addon. Disable Polymer on YouTube is currently the only one of these addons that still successfully reverts the UI across the board, in my testing (YMMV).
It works by adding the parameter "disable_polymer=true" to all YouTube URLs. One of the competing addons works like that as well, but inconsistently, and apparently only on video URLs; the others no longer work properly at all (e.g. due to trying to manipulate the PREF cookie). Who knows how long this final method will last of getting the far superior old UI, but I'll continue to dig in my claws until utterly forced to use the functionally degraded "modern" interface.
It's interesting that a couple of other people report that this addon isn't working for them anymore as of yesterday. The current version was released in mid-2018, so an update can't explain my different results. I did start a fresh Firefox profile for unrelated reasons, so I didn't have any old youtube.com cookies lying around that might break the disable_polymer workaround. Those who are having trouble, first, please let us know which country's YouTube site you're using, as that may make a difference. Second, please try clearing all your youtube.com (and/or appropriate YouTube international site) cookies to see if that fixes it.
One thing that some people are complaining about is that the "show more" link for long Descriptions no longer shows up in the classic UI. This is YouTube's fault, not the fault of addons that revert to the pre-Polymer UI. Luckily, this can currently be easily fixed by adding a custom uBlock Origin filter rule to auto-expand the description:

See the reviews by CHAZZ and tanzie of the YouTube Classic addon for more details on that. Hopefully the author of one of the Polymer-reverting extensions will incorporate that transformation into their addon, so the extra step won't be necessary for long descriptions to be readable. - Valutata 5 su 5di Cosmic_Annihilation, 4 anni faUnfortunately this stopped working as of today as Youtube made some changes to further deprecate the old UI. It's still possible to bring back the old interface by changing the PREF value in the yt cookie but users are presented with the message "This version of YouTube is going away soon"
